The Rehab Team
Our team of licensed physical therapists, occupational therapists, and speech language pathologists work with our nurses, social workers, dietitians, and activity personnel to meet each person’s medical needs and goals. Implementing an individualized plan of care approved by the physician, our therapists use best practices and evidence-based procedures to achieve the best outcome possible. The therapy gyms offer an inviting environment to help our patients regain skills necessary to improve their function.

Post-Acute Care
Not everyone is able to discharge directly from the hospital to home. Sunrise Health and Rehabilitation Center provides short-term skilled nursing and rehabilitation services to help patients recover in order to transition from the hospital to their home or alternate living situation. Therapists are involved to help patients every step of the way and provide support for smooth transition.
